What is personal information?

CHS is committed to protecting your personal information. The term “personal information” refers to information that specifically identifies you as an individual which could include: age, name, address, gender, weight and height, ID numbers, place of birth, ethnic origin, employment or financial history, opinions, evaluations, comments about an individual.

Personal information does not include the name, title, business address or telephone number of an employee of an organization.

Why do we collect personal information?

CHS collects personal information through two main avenues:

1)    Through various CHS forms, i.e.  workshop registration forms, order forms; and
2)    When you access the website.

What do we do with this personal information?

CHS uses your personal information to complete a transaction, to communicate with you, to provide you with service and benefits, and to improve or add to services and benefits.

Credit card numbers are used only for payment processing, and are not retained by CHS.  Occasionally, CHS may use your information to contact you for quality assurance or satisfaction information.  CHS will not sell, trade, rent or lease your “personally identifiable information” to others, unless with your consent.

How do we protect your personal information? 

CHS takes your privacy and security seriously. To prevent unauthorized access, maintain data accuracy and ensure the appropriate use of information, we have put in place physical, electronic and managerial procedures to safeguard and secure the information you provide to us.

How to correct personal information?

If you notice that the personal information that CHS has collected requires correction e.g. change of address, contact CHS to make the appropriate changes.
